According to CoinMeta, Claude discovered a historical record Bug while managing a batch of Codex. A user's session lasted for two consecutive days, leaving behind 110MB of records and approximately 353 million token in use. However, when the session was reopened, Codex could only recall the first sentence. The actual content of the conversation was not lost; Codex stores the complete chat records locally and creates an index for quick retrieval. The issue lay with the index, which became stuck at an early point, meaning that although subsequent records existed, Codex could not be accessed. The user checked 439 sessions, and 398 of them had abnormal indexes. Since then, similar "false amnesia" phenomena have been reported in environments such as Windows, desktop, remote, and ssh. After the original poster deleted the damaged index, Codex was able to read a 110MB session in about 20 seconds and restored all 398 abnormal sessions. Currently, this Bug issue is still unresolved, and no related fixes have been observed yet.
